An attempt was made to evaluate your property for the purpose of issuing an Existing Tank Permit.
The evaluation could not be completed for one or more of the following reasons.
1. Uncover outlet lid on septic tank and break seal ($25.00 fee incurred)
2. Have system pumped out

_X_ 4. Other - You will need to revise your application and site plan at Central Permitting's
office to change your setbacks for the home. It should be moved forward a minimum of 3 feet.
