IF THE INFORMATION IN THIS APPLICATION IS FALSIFIED, CHANGED, OR THE SITE 1S ALTERED, THEN THE IMPROVEMENT
PERMIT OR AUTHORIZATION TO CONSTRUCT SHALL BECOME INVALID. The permit is valid for either 60 months or without expiration

• All property Irons must be made visible. Place "pink property flags" on each corner iron of lot. All property
lines must be clearly flagged approximately every 50 feet between corners.
• Place "orange house corner flags" at each corner of the proposed structure. Also flag driveways, garages, decks,
out buildings, swimming pools, etc. Place flags per site plan developed al/for Central Permitting.
• Place orange Environmental Health card in location that is easily viewed from road to assist in locating property.
• If property is thickly wooded, Environmental Health requires that you clean out the undergrowth to allow the soil
evaluation to be performed. Inspectors should be able to walk freely around site. Do not grade property.
• All lots to be addressed within 10 business days after confirmation. $25.00 return trip fee may be incurred
for failure to uncover outlet lid. mark house comers and property lines. etc. once lot confirmed ready.
• After preparing proposed site call the voice permitting system at 910 - 893 -7525 option 1 to schedule and use code
,800 (after selecting notification permit if multiple permits exist) for Environmental Health inspection. Please note
confirmation number given at end of recording for proof of request.
Use Click2Gov or IVR to verify results. Once approved, proceed to Central Permitting for permits.

• Follow above instructions for placing flags and card on property.
• Prepare for inspection by removing soil over outlet end of tank as diagram indicates, and lift lid straight up (if
possible) and then put lld back Inplace. (Unless inspection is for a septic tank in a mobile home park)
DO NOT LEAVE LIDS OFF OF SEPTIC TANK
• After uncovering outlet end call the voice permitting system at 910- 893 -7525 option 1 & select notification permit
if multiple permits, then use code 800 for Environmental Health inspection. Please note confirmation number
given at end of recording for proof of request.
• Use Click2Gov or IVR to hear results. Once approved, proceed to Central Permitting for remaining permits.

Rental /Lease Agreement
1. This agreement Is between HoCar Properties, LLC, hereafter referred to as "Landlord ", and

2. Tenant agrees to lease /rent the described mobile home lot #6, lot #25, lot #26, lot #27 in Eagles
Landing Mobile Home Park, Broadway, NC, 27505 for the sum of $125.00 a month each.
Payments shall begin on March 1 ", 2011 and shall be due on the 1" of each month thereafter.
4. Payments not paid by the 15 "' of each months shall be subject to a 555.00 late fee. Failure to
pay the late fee shall be the same as failure to pay the original rent. HoCar Properties will begin
the eviction process on the 16Th of the month unless prior arrangements have been made.
5. Tenant agrees to pay a N/A security deposit that shall be refundable provided the tenant gives
at least a 30 day notice (after Initial 12 month period) of moving and the property Is clean and In
the same condition as when tenant moved in. The security deposit Is non refundable If tenant
owes landlord any back rent.
6. Tenant agrees that he/she will keep the lot In good condition to Include keeping grass mowed
and lot clean. Tenant shall not have trash, junk cars or appliances, large or vicious animals or any
other activity that could be considered a nuisance by neighbors, including loud parties or music
and excessive traffic in and out of the home.
7. Tenant agrees that he/she will not possess, sale or use illegal drugs on described property, or
allow any other guest or visitor to possess, sale or use any illegal products. Tenant also agrees
that he /she shall not use the home for any other activity that Is In violation of state or federal
laws.
8. Animals kept outside the home must be kept In a manner that complies with any local animal
control ordinances and In a manner that does not Intrude upon neighbors.
9. HoCar properties shall provide lot and trash pick up only
