The sewer permit WQ0029771 was issued to HCDPU for this development with the
understanding that all HCDPU requirements would be satisfied and this permit may be
rescinded with NCDENR-DWQ if the established HCDPU Standards and Specifications are not
followed for the design, installation, testing, inspection and approval of the proposed sewer
collections lines.
The cost estimate certified by Robert J. Bracken, P.E. for the performance bond does not
include cost for water line extensions to serve the subject project. The performance bond must
include the cost to install the proposed water lines.
The cost estimate certified by Robert J. Bracken, P.E. for the performance bond does not
include cost for a parschal flume and flow meter at the existing lagoon where the proposed
force main will discharge. The performance bond must include the cost to install the parschal
flume and flow meter.
The cost estimate certified by Robert J. Bracken, P.E. for the performance bond does not
include cost for Water Capacity Use Fees, Sewer Capacity Use Fees, Meter Fees, or SCADA Fees
that will be required by HCDPU for this proposed subdivision. The performance bond must
include the cost to of all applicable fees imposed by HCDPU for this subdivision.
Revise the overall Cost Estimate for the improvements to include all utilities and applicable fees
as required and resubmit the cost estimate for the performance bond to HCDPU.
Robert J. Bracken, P.E. must submit design calculations for the parschal flume and flow meter
and revise the utility plans to reflect the connection to the existing metering station at the
Carolina lakes WWTP.
Robert J. Bracken, P.E. must submit all maps, As-Built Record Drawings and PE Certifications
for all previous phases of the Carolina Lakes Development to HCDPU before the final plat will be
approved and the project will be placed on Hold until these documents have been provided to
HCDPU as required.

The Developer will be required to provide HCDPU with a one (1) year warranty on the water
and sewer lines installed to serve this subdivision once these utilities have been inspected,
tested and approved by HCDPU, Final Acceptance by HCDPU and Final Approvals by NCDENR.
The Developer or the Utility Contractor must provide HCDPU with the Cost Statement of the
water and sewer improvements installed to serve this subdivision.

The Developer must provide HCDPU with a Master Development Plan for any and all additional
phases to be developed in this subdivision in accordance with the approval process of HCDPU.
The proposed water lines have been permitted by NCDENR-DEH,PWSS for HCDPU under the
DEH permit #06-01645 and it will expire if construction does not begin before 10-13-2008.
