An attempt was made to evaluate your property for the purpose of issuing an Existing Tank Permit.
The evaluation could not be completed for one or more of the following reasons.
1. Uncover outlet lid on septic tank and break seal ($25.00 fee incurred)
2. Have system pumped out
3. Replace sanitary T

outlet lid of the septic tank (wrong lid was uncovered). Locate and uncover the distribution box.
Locate and expose the drain field due to the additional bedroom and due to the location of the
addition in itself in relation to your existing drain field. A recorded survey map is required.
(Can't use the GIS aerial photo you provided to accurately determine your property line
dimensions). You may need to demonstrate your property irons as well. Call me to discuss
further or if you have any questions.
Your application will be put on hold until the selected items above have been addressed. When
completed call 893 -7547 to confirm that the items mentioned have been corrected, we will then
reschedule your property for evaluation.
